An evaluation of governmental homes for the aged: case studies of the three homes for the aged Abstract A summary of the resource. การศึกษาวิจัยเรื่องนี้เป็นการประเมินสถานสงเคราะห์คนชราของรัฐ ศึกษาเฉพาะกรณีสถานสงเคราะห์คนชรารวม 3 แห่ง โดยใช้ระเบียบวิธีวิจัยเชิงปริมาณผสมผสานกับเชิงคุณภาพ กรอบแนวคิดที่ใช้นั้นได้ดัดแปลงมาจากรูปแบบการประเมิน CIPP และมีการนำมิติหญิงชายมาวิเคราะห์ข้อมูลร่วมด้วย ผลการศึกษาจากการประเมินบริบทแวดล้อมของสถานสงเคราะห์คนชรา พบว่าพื้นที่ของสถานสงเคราะห์และสภาพแวดล้อมมีผลต่อปฏิสัมพันธ์และพฤติกรรมของผู้สูงอายุและพบด้วยว่าชุมชนใกล้เคียงสถานสงเคราะห์และสาธารณชน ยังไม่มีส่วนร่วมในการดำเนินงานและการตรวจสอบการดำเนินงานของสถานสงเคราะห์คนชราแต่อย่างใด ในด้านการประเมินผู้สูงอายุในสถานสงเคราะห์คนชรา พบว่า ผู้สูงอายุที่เป็นกลุ่มตัวอย่างส่วนใหญ่เป็นหญิงมากกว่าชาย และมีฐานะทางเศรษฐกิจและสังคมอยู่ในระดับล่าง สาเหตุที่เข้ามาอยู่ในสถานสงเคราะห์ก็เพราะไม่มีผู้ดูแลอยู่ที่บ้าน ทั้งนี้ผู้สูงอายุในสัดส่วนที่สูงไม่เข้าร่วมกิจกรรมใดๆ ของสถานสงเคราะห์เลยเพราะปัญหาสุขภาพ อย่างไรก็ดีผู้สูงอายุส่วนใหญ่ก็มีทัศนะว่าชีวิตในสถานสงเคราะห์ปัจจุบันดีขึ้นกว่าแต่ก่อน ซึ่งผลของการศึกษายังพบแนวโน้มที่ผู้สูงอายุจะมีการศึกษาและมีฐานะเศรษฐกิจสังคมสูงขึ้น ทำให้สถานสงเคราะห์ต้องเผชิญกับปัญหาการเรียกร้องสิทธิของผู้สูงอายุเพิ่มขึ้นเรื่อยๆ เมื่อนำมิติหญิงชายมาวิเคราะห์ ก็พบว่าผู้สูงอายุชายมีฐานะทางเศรษฐกิจและสังคมดั้งเดิมดีกว่าผู้หญิง มีรายได้ปัจจุบันมากกว่าผู้หญิง สุขภาพร่างกายแข็งแรงมากกว่า และมีเครือญาติมากกว่าผู้หญิง อีกทั้งยังเข้าร่วมกิจกรรมมากกว่าผู้หญิงด้วย ในด้านจิตใจพบว่าผู้สูงอายุชายมีสุขภาพจิตดีกว่าผู้สูงอายุหญิง มีความต้องการทางจิตใจและจิตวิญญาณน้อยกว่าผู้หญิง ส่วนในด้านศักยภาพก็พบว่า แม้สัดส่วนของผู้หญิงที่มีความถนัดหรือความชำนาญจะสูงกว่าผู้ชาย แต่เมื่อเปรียบเทียบกันแล้ว ผู้ชายต้องการทำประโยชน์ให้แก่ชุมชนและสังคมมากกว่าผู้หญิง และในประการสุดท้าย ผู้สูงอายุหญิงต้องการอยู่ในสถานสงเคราะห์คนชราตลอดไปมากกว่าผู้ชาย ผลประเมินในด้านทรัพยากรในการบริหารงานของสถานสงเคราะห์ พบว่ามีการใช้จ่ายทรัพยากรไปในการสงเคราะห์ผู้สูงอายุต่อบุคคลค่อนข้างสูงถึงปีละ 34,500 บาท ในด้านนโยบายพบว่านโยบายของสถานสงเคราะห์คนชราต่างๆ ในสังกัดกรมประชาสงเคราะห์ยังขาดเอกภาพ เพราะยึดถือกรอบนโยบายหลายกรอบ และแผนงานกับโครงการก็มีความยืดหยุ่นสูงเกินไป อีกทั้งยังไม่มีกลไกในการติดตามประเมินผลการดำเนินงานตามแผนงานกับโครงการดังกล่าวด้วย ในด้านการบริหารงานก็พบว่า สถานสงเคราะห์คนชราในต่างจังหวัดมีความเป็น “อาณาจักร” สูง และยึดโยงกับส่วนกลางมากจนยังมิได้เป็นหน่วยงานในส่วนภูมิภาคอย่างแท้จริง ส่วนในด้านการบริหารงบประมาณ ก็พบว่าสถานสงเคราะห์แต่ละแห่งยังต้องพึ่งพาการรับบริจาคจากสาธารณชนมาก และในด้านการบริหารบุคลากรก็พบว่า การกระจายตัวของบุคลากรในแต่ละสถานสงเคราะห์ยังไม่สมดุล และไม่เป็นไปตามกรอบอัตรากำลังที่ตั้งไว้ ในด้านการให้บริการของสถานสงเคราะห์ พบว่ามีการรับผู้สูงอายุที่ไม่เดือดร้อนจำเป็นจริง ๆ เข้าสถานสงเคราะห์คนชรา และพบด้วยว่าจุดแข็งของสถานสงเคราะห์ ก็คือ สามารถตอบสนองความต้องการด้านกายภาพของผู้สูงอายุได้เป็นอย่างดี แต่ข้อจำกัดก็คือไม่อาจตอบสนองความต้องการด้านจิตใจและจิตวิญญาณของผู้สูงอายุได้ ส่วนในด้านการปฏิบัติงานของบุคลากร ก็พบว่าปัญหาสำคัญได้แก่บุคลากรระดับล่างยังไม่มีคุณภาพเท่าที่ควร และ บุคลากรสถานสงเคราะห์เองก็ยังไม่อาจจะแก้ไขปัญหาของผู้สูงอายุในสถานสงเคราะห์ ที่นับวันก็จะทวีความรุนแรงมากขึ้นได้อย่างมีประสิทธิภาพ ในด้านผลการดำเนินงานของสถานสงเคราะห์ พบว่าบริการและกิจกรรมมีความพอเพียงแล้ว และเจ้าหน้าที่ก็ปฏิบัติต่อผู้สูงอายุอย่างเสมอภาคและเป็นธรรม แต่สิ่งที่ไม่เป็นธรรม ก็คือผู้สูงอายุที่มีฐานะดีและปานกลางค่อนข้างดี ได้รับประโยชน์จากสถานสงเคราะห์เท่าเทียมกับผู้สูงอายุที่ยากจน ไร้ที่พึ่งพา และการที่ผู้สูงอายุถดถอยยังเข้าไม่ถึงบริการกับกิจกรรมของสถานสงเคราะห์เท่าที่ควร ส่วนในด้านความยั่งยืนของบริการและกิจกรรม ก็พบว่าผู้สูงอายุเป็นผู้รอรับบริการมากกว่าการมีส่วนร่วม จึงยังไม่อาจเรียกได้ว่ามีความยั่งยืนอย่างแท้จริงได้ ในประการสุดท้าย คือ ผลกระทบที่เกิดจากสถานสงเคราะห์นั้น พบว่าการที่ผู้สูงอายุและครอบครัวยากจนได้รับประโยชน์จากการที่ผู้สูงอายุเข้ามารับการสงเคราะห์ จัดได้ว่าเป็นผลกระทบทางตรง ส่วนผลกระทบทางอ้อมที่สำคัญ ก็คือ การที่สถานสงเคราะห์เป็นจุดให้กำเนิดของศูนย์บริการผู้สูงอายุในชุมชน http://digital.nlt.go.th/dlib/files/original/289da8b94127f5d6edbf674979896a9d.pdf 6b9f3d906ab361f9a7742ef640c77883 Dublin Core The Dublin Core metadata element set is common to all Omeka records, including items, files, and collections. Management of expensive medical-technology equipment: the literature review at the policy lever Abstract A summary of the resource. การจัดการเทคโนโลยีทางการแพทย์ประเภทครุภัณฑ์ราคาแพง: การทบทวนองค์ความรู้ระดับนโยบาย การศึกษานี้เป็นงานวิจัยเอกสาร มีวัตถุประสงค์เพื่อศึกษาทบทวนข้อมูลและวรรณกรรมเชิงวิชาการด้านเศรษฐศาสตร์สาธารณสุข และการจัดการเทคโนโลยีทางการแพทย์ที่มีการศึกษาในประเทศไทย รวบรวมข้อเสนอแนะจากงานวิจัยเชิงเศรษฐศาสตร์สาธารณสุขและงานวิจัยเชิงนโยบายเกี่ยวกับเทคโนโลยีทางการแพทย์ประเภทครุภัณฑ์ที่มีราคาแพง สถิติ ความคิดเห็น หรือทางเลือกเชิงนโยบายที่มีผู้รวบรวมไว้ในงานบทความวิชาการ ตลอดจนศึกษาทบทวนองค์ความรู้เชิงวิชาการ ที่เกี่ยวข้องกับการจัดการเทคโนโลยีทางการแพทย์ประเภทครุภัณฑ์ที่มีราคาแพงในระดับนโยบายสาธารณะ (Public policies) และนโยบายระบบ (System policies) ที่มีการศึกษาและเผยแพร่ในวารสารวิชาการต่างประเทศเกี่ยวกับการประเมิน การจัดหา จัดซื้อ การกระจาย ต้นทุนค่าใช้จ่ายและการใช้ประโยชน์เทคโนโลยีทางการแพทย์ ผลการศึกษาพบว่า ประเทศไทยยังมีการศึกษาวิจัยเกี่ยวกับการจัดการเทคโนโลยีทางการแพทย์ประเภทครุภัณฑ์ราคาแพงไม่มากนัก การศึกษาส่วนใหญ่เน้นไปที่เทคโนโลยีเฉพาะเรื่อง มีการศึกษาเพื่อประเมินเทคโนโลยี (Technology assessment) อยู่บ้าง แต่ยังไม่กว้างขวาง การลงทุนทั้งในภาครัฐและเอกชนในช่วงก่อนวิกฤติเศรษฐกิจในด้านครุภัณฑ์ทางการแพทย์มีแนวโน้มสูงขึ้นตลอดเวลา ในขณะที่การเก็บสถิติการกระจายของเทคโนโลยียังไม่สมบูรณ์ มีหลักฐานบ่งชี้ว่าการเข้าถึงเทคโนโลยีของประชาชนอาจไม่มีความเสมอภาค ซึ่งประเทศยังไม่มีองค์กรหรือกลไกในการจัดการเกี่ยวกับเทคโนโลยีทางการแพทย์ได้อย่างครบวงจรและมีประสิทธิผล อีกทั้งความรับผิดชอบในการกำกับดูแลกระจายอยู่ในหน่วยงานของรัฐหลายหน่วยงาน การทบทวนวรรณกรรมในต่างประเทศในเรื่องการประเมิน การจัดหา จัดซื้อ การกระจาย ต้นทุนค่าใช้จ่ายและการใช้ประโยชน์เทคโนโลยีทางการแพทย์ประเภทครุภัณฑ์ที่มีราคาแพงพบว่ามีรายละเอียดของข้อมูลในแต่ละประเด็นกล่าวถึงไว้ในแต่ละประเด็นไม่เท่ากัน อย่างไรก็ตามอาจพอนำมาสรุปรวบยอดได้ว่า มาตรการในการจัดการเทคโนโลยีทางการแพทย์แบ่งได้เป็น 3 ระยะใหญ่ๆคือ (1) มาตรการระยะก่อนเข้าสู่ตลาด (Pre-market) ได้แก่ มาตรการที่ต้องปฏิบัติก่อนที่จะมีการนำเทคโนโลยีมาใช้ และเผยแพร่ในประเทศ การประเมินความต้องการ การประเมินความพร้อมในด้านโครงสร้างที่จะรองรับการใช้ประโยชน์ และการประเมินเทคโนโลยีนั้นเอง (2) มาตรการต่อกระบวนการจัดหาและการกระจายของเทคโนโลยี (Purchasing and diffusion) ได้แก่ มาตรการเมื่อเทคโนโลยีทางการแพทย์ได้รับการประเมินแล้วว่าปลอดภัย มีความเหมาะสมที่จะนำมาใช้ได้ในประเทศ เพื่อให้มีการกระจายเทคโนโลยีไปอย่างเหมาะสม ให้เกิดประสิทธิภาพและประสิทธิผล ตลอดจนความเสมอภาคมากที่สุด และ (3) มาตรการต่อการใช้เทคโนโลยี (Use) ได้แก่ มาตรการที่มีผลต่อการตัดสินใจของแพทย์ในการใช้ประโยชน์จากเทคโนโลยีในการตรวจวินิจฉัยหรือรักษาคนไข้ เพื่อให้ใช้เทคโนโลยีเป็นไปอย่างเหมาะสมที่สุด ไม่ใช้เทคโนโลยีมากหรือน้อยเกินไป http://digital.nlt.go.th/dlib/files/original/9c9fc569fbd7ed01a014093c69124260.pdf f89faed5bf55a37ca3e04fdcc66d7211 Dublin Core The Dublin Core metadata element set is common to all Omeka records, including items, files, and collections.
