Potential sources of finance for the universal health care coverage fund by the article 39 of the 2002 national health security act : Key informants perspective Abstract A summary of the resource. แหล่งการเงินของโครงการหลักประกันสุขภาพถ้วนหน้าในขณะนี้มาจากการจัดทำคำขอจากเงินงบประมาณรายจ่ายประจำปีของรัฐบาลโดยเลือกใช้การจัดทำงบประมาณปลายปิดด้วยอัตราเหมาจ่ายรายหัวคูณกับจำนวนประชากรที่ขึ้นทะเบียน จากประสบการณ์ที่ผ่านมา พบว่า กระบวนการนี้ต้องลงทุนลงแรงในการจัดทำอย่างมาก โดยที่ไม่มีหลักประกันใดๆว่าจะได้รับอนุมัติงบประมาณอย่างเพียงพอตามอัตราเหมาจ่ายที่เหมาะสมกับต้นทุนและอัตราการใช้บริการในแต่ละปี สำนักงานหลักประกันสุขภาพแห่งชาติและประชาชนควรต้องจัดให้มีกองทุนหลักประกันสุขภาพแห่งชาติขึ้นตาม พระราชบัญญัติหลักประกันสุขภาพแห่งชาติ พ.ศ. 2545 โดยมีแหล่งเงินหลักที่ไม่ผูกขาดอยู่กับเงินงบประมาณประจำปีของรัฐบาลเพียงแหล่งเดียวดังเช่นที่เป็นอยู่การศึกษาครั้งนี้มีวัตถุประสงค์เพื่อเสนอข้อดีและข้อจำกัดของแหล่งเงินต่างๆของกองทุนจำนวน 8 แหล่งเงินตามที่ระบุในมาตรา 39 พรบ.หลักประกันสุขภาพแห่งชาติ ด้วยสองวิธีการ ได้แก่การระดมสมองนักวิจัยในทีมงาน และ การสอบถามความเห็นจากผู้สันทัดกรณีด้วยแบบสอบที่สร้างขึ้นใหม่โดยการจัดประชุมเชิงปฏิบัติการ ผลการศึกษาจากการระดมสมองของนักวิจัยพบว่า มาตรา 39 (2) เงินจากองค์กรปกครองท้องถิ่น, (3) การเก็บเงินจากผู้ใช้บริการ และ (8) การเก็บเงินสมทบ มีศักยภาพในการที่จะเป็นแหล่งการเงินของกองทุนฯได้ แต่มีข้อจำกัดของแต่ละแหล่งเงินที่ยากในการเป็นแหล่งเงินหลักของกองทุนฯ ผลจากแบบสอบถามความคิดเห็นผู้สันทัดกรณีจำนวน 20 ชุด พบว่า ความเป็นไปได้ทางการเมืองเป็นประเด็นสำคัญที่สุดต่อการตัดสินใจเลือกแหล่งการเงินให้กองทุน ปัจจัยรองลงมา ได้แก่ ความยั่งยืนของแหล่งการคลัง ความเสมอภาคในสังคม ความเป็นไปได้ในทางปฎิบัติ ส่วนการยอมรับของสังคมเป็นประเด็นที่ได้รับความเห็นว่ามีความสำคัญน้อยที่สุด ผลจากการให้ค่าคะแนนความเป็นไปได้ตามประเด็นทั้งห้าข้างต้นในแต่ละแหล่งการคลังที่ถ่วงน้ำหนักตามความสำคัญของแต่ละประเด็นแล้ว พบว่า การเพิ่มภาษีบุหรี่ เหล้า และเบียร์ เป็นแหล่งที่มีความเป็นไปได้มากที่สุด จากความเป็นไปได้ทางการเมืองมีมาก การยอมรับทางสังคมสูง มีความยั่งยืนในการเป็นแหล่งเงิน และ การดำเนินการทำได้ง่าย ในทางกลับกัน การเก็บเงินสมทบจากผู้มีสิทธิ์มีความเป็นไปได้น้อยที่สุด เนื่องจากการยอมรับของสังคมน้อย และ ความยุ่งยากในการดำเนินการ นอกจากนี้ยังพบว่าการเก็บภาษีมูลค่าเพิ่มมีความเป็นไปได้น้อยที่สุดในทางการเมืองและเป็นทางเลือกที่แย่ที่สุดในด้านความเสมอภาค แบบสอบถามที่ใช้เป็นแบบสอบถามที่ถูกพัฒนาขึ้นใหม่จากพื้นฐานความรู้ ความเข้าใจการเงินการคลังระบบสุขภาพไทย กระบวนการตอบแบบสอบถามเลือกใช้การประชุมเชิงปฏิบัติการที่มีการให้ข้อมูลแก่ผู้ตอบแบบสอบถามอย่างรอบด้าน เพราะต้องทำความเข้าใจให้ตรงกัน จึงเป็นข้อจำกัดที่ทำให้ได้ตัวอย่างแบบสอบถามจำนวนจำกัด แต่ก็เป็นตัวอย่างที่มาจากนักวิชาการหลากหลายสถาบันทั้งในองค์กรวิชาการและภาคปฏิบัติ ผลที่ได้จากการศึกษาในครั้งนี้ให้ความรู้เชิงประจักษ์ในเรื่องทางเลือกแหล่งการเงินการคลังของกองทุนฯ ซึ่งถือได้ว่าเป็นความรู้ใหม่อย่างหนึ่งของแหล่งการเงินการคลังในระบบสุขภาพไทย นอกจากความรู้ใหม่ทางวิชาการที่ได้แล้ว ผลการศึกษาครั้งนี้ยังเป็นข้อมูลเบื้องต้นสำหรับการศึกษาแหล่งการคลังของกองทุนฯ ในระยะยาวต่อไปด้วย http://digital.nlt.go.th/dlib/files/original/b191380d8a7306d71e8af229401cbd97.pdf 7955003015f6c3476c1537c9d22938c0 Dublin Core The Dublin Core metadata element set is common to all Omeka records, including items, files, and collections.
