A study of a model and evaluation of an autonomous district hospital on administration and management with in the hospital : a case study of Banpaew hospital Abstract A summary of the resource. โรงพยาบาลบ้านแพ้ว จังหวัดสมุทรสาคร เป็นโรงพยาบาลแห่งแรกที่ออกนอกระบบราชการ เป็นโรงพยาบาลในกำกับของรัฐเมื่อวันที่ 1 ตุลาคม 2543 เก็บค่ารักษาพยาบาลคนละ 40 บาท สำหรับผู้ป่วยนอก และ 100 บาท สำหรับผู้ป่วยใน จึงสมควรศึกษาผลกระทบที่มีต่อสุขภาพประชาชนในอำเภอบ้านแพ้ว โดยมีวัตถุประสงค์ เพื่อศึกษาการเปลี่ยนแปลงภาวะสุขภาพ ทัศนคติ พฤติกรรมการดูแลสุขภาพ ปัจจัยทางสังคมและสิ่งแวดล้อม และการเปลี่ยนแปลงของการใช้บริการทางสุขภาพของประชาชนในอำเภอบ้านแพ้ว เปรียบเทียบก่อนและหลังจากที่โรงพยาบาลบ้านแพ้วเป็นโรงพยาบาลในกำกับของรัฐ โดยใช้รูปแบบการศึกษากึ่งทดลอง (Quasi experimental study) สุ่มตัวอย่างแบบแบ่งชั้นพื้นที่หลายชั้น (Stratified Multistage Sampling) อำเภอบ้านแพ้วมี 12 ตำบลแบ่งเป็นในเขตเทศบาล และนอกเขตเทศบาล แต่ละเขตสุ่มหมู่บ้าน แต่ละหมู่บ้าน สุ่ม 40 ครัวเรือน รวมทั้งหมด 1,000 ครัวเรือน สมาชิกเฉลี่ยครัวเรือนละ 4-6 คน มีสมาชิกรวมทั้งหมด 4,485 คน การเก็บข้อมูลใช้วิธีการสัมภาษณ์ การสังเกตสถานภาพครัวเรือน สิ่งแวดล้อม และสมาชิกในครัวเรือนทุกคน ดำเนินการสำรวจ 2 ครั้ง ครั้งแรกหลังจากออกนอกระบบปีที่ 1 เมื่อเดือน มีนาคม – เมษายน 2544 ครั้งที่ 2 เมื่อเดือน สิงหาคม – กันยายน 2545 และในเดือนตุลาคม 2544 รัฐบาลนำนโยบาย 30 บาทรักษาทุกโรคมาใช้ในจังหวัดสมุทรสาครด้วย ผลการศึกษาจากการสำรวจ 2 ครั้ง การศึกษาในทุกด้านอยู่ในเกณฑ์ที่ดี เนื่องจากการสำรวจครั้งที่ 2 ได้มีการเปลี่ยนนโยบายด้านระบบสาธารณสุขใหม่ โดยคิดอัตราค่าบริการ 30 บาท ทั้งผู้ป่วยนอกและผู้ป่วยใน ทำให้ค่าใช้จ่ายที่เป็นเวชภัณฑ์ และยาที่ไม่สามารถเบิกได้ เฉลี่ยลดต่ำลงกว่า การสำรวจครั้งแรกอย่างมีนัยสำคัญ ประชาชนไปใช้บริการที่โรงพยาบาลบ้านแพ้วในฐานะผู้ป่วยใน และผู้ป่วยนอก และมีความพึงพอใจ การให้บริการทั้งคุณภาพ ประสิทธิภาพ และบุคลากรของโรงพยาบาลมากกว่าการสำรวจครั้งแรก (P > .05) สำหรับบัตรประกันสุขภาพของสมาชิกในครัวเรือน ในการสำรวจครั้งที่ 2 มีถึงร้อยละ 95 เพราะมีนโยบาย 30 บาทรักษาทุกโรค ขณะที่การสำรวจครั้งแรกมีเพียงร้อยละ 49 เมื่อประชาชนเจ็บป่วยจะไปรับบริการที่โรงพยาบาลของรัฐ และสถานีอนามัย สุขภาพประชาชนอยู่ในเกณฑ์ดีถึงดีมาก ทั้งสองช่วงเวลาของการสำรวจผู้ที่เจ็บป่วยส่วนมากจะเป็นผู้สูงอายุ 60 ปีขึ้นไป เป็นโรคเบาหวาน โรคความดันสูง และโรคข้อเสื่อม อักเสบ และพบในการสำรวจครั้งที่ 2 มากกว่าการสำรวจครั้งแรก (P < 0.5) เนื่องจากมีบุคลากรสาธารณสุขไปดูแลเยี่ยมบ้านให้คำแนะนำและให้ความรู้ในชุมชนมากขึ้น ประชาชนดูแลสุขภาพตนเองตรวจและส่งเสริมสุขภาพมากขึ้น โดยใช้บริการโรงพยาบาลบ้านแพ้ว และสถานีอนามัยมากขึ้นกว่าการสำรวจครั้งแรก ( P < .05) อนึ่ง การศึกษาครั้งนี้เป็นการศึกษาในระยะเริ่มต้นเพียง 2 ปี หลักการเปลี่ยนแปลงสถานภาพของโรงพยาบาลบ้านแพ้วและมีนโยบาย 30 บาทรักษาทุกโรคเข้ามาใช้เมื่อตุลาคม 2544 จึงควรติดตามผลในระยะยาว 4-5 ปี จะประเมินได้เหมาะสมถูกต้องมากยิ่งขึ้นคำสำคัญ : วิจัยการให้บริการสุขภาพ, การวิจัยกึ่งทดลองโรงพยาบาลในกำกับของรัฐ
