Consumer protection system in Thai health insurance Abstract A summary of the resource. การพัฒนาระบบการคุ้มครองผู้บริโภคไม่เพียงแต่จะทำให้ประชาชนสามารถใช้บริการสุขภาพได้อย่างมีประสิทธิภาพเท่านั้น กลับจะเป็นการกระตุ้นให้เกิดการพัฒนาคุณภาพบริการในทางอ้อมได้อีกด้วย ในการศึกษานี้มีวัตถุประสงค์เพื่อประมวลสถานการณ์การคุ้มครองผู้บริโภคในระบบประกันสุขภาพของไทย และจัดทำข้อเสนอแนะเชิงนโยบายเพื่อพัฒนาระบบดังกล่าว โดยที่ระบบประกันสุขภาพที่ศึกษาประกอบด้วยระบบหลักประกันสุขภาพถ้วนหน้า(30 บาทรักษาทุกโรค) ระบบประกันสังคม และระบบสวัสดิการรักษาพยาบาลของข้าราชการและพนักงานรัฐวิสาหกิจ โดยมีกรอบการศึกษาเพื่อประเมินในเรื่องสิทธิของประชาชนใน 5 ด้านคือ สิทธิในการเข้าถึงบริการสุขภาพที่จำเป็น สิทธิในการได้รับบริการที่มีคุณภาพ สิทธิในการได้รับบริการที่ครบถ้วนตามชุดสิทธิประโยชน์ สิทธิในการเข้าร่วมบริหารจัดการระบบประกันสุขภาพ และสิทธิในการรับรู้ข้อมูลที่จำเป็น วิธีการศึกษาประกอบด้วย การประชุมอภิปรายกลุ่มผู้เกี่ยวข้องเพื่อรวบรวมข้อมูลสถานการณ์และข้อเสนอเชิงนโยบาย การสำรวจความคิดเห็นของประชาชนต่อระบบหลักประกันสุขภาพแห่งชาติโดยใช้แบบสอบถาม การศึกษาเชิงเอกสารเพื่อประมวลข้อมูลที่จำเป็นสำหรับประชาชนในระบบประกันสุขภาพและได้มีการส่งผลการศึกษาเบื้องต้นไปยังผู้แทนภาคประชาชนในคณะอนุกรรมการหลักประกันสุขภาพประจำจังหวัดเพื่อให้ข้อคิดเห็นเพิ่มเติมเพื่อปรับปรุงรายงานฉบับสมบูรณ์ ในการศึกษาได้ผลที่สำคัญดังนี้ ยังพบว่ามีประชาชนที่ยังไม่สามารถเข้าถึงระบบประกันสุขภาพเนื่องจากไม่มีหลักฐานการแสดงสิทธิการเป็นคนไทย ประชาชนมีความเชื่อมั่นต่อคุณภาพบริการและยาค่อนข้างน้อยโดยเฉพาะเมื่อจำเป็นต้องได้รับบริการสุขภาพหรือยาที่มีราคาแพง ประชาชนกลุ่มเฉพาะได้แก่ ผู้พิการ ผู้ติดเชื้อHIV และผู้ป่วยเอดส์ และผู้ใช้แรงงานนอกระบบทางการไม่ได้รับบริการที่ครบถ้วนตามที่กำหนดในชุดสิทธิประโยชน์ระบบฐาน ข้อมูลที่จำเป็นสำหรับประชาชนยังไม่เพียงพอ และระดับการมีส่วนร่วมของประชาชนในการบริหาร จัดการระบบประกันสุขภาพอยู่ในระดับต่ำ ในการศึกษามีข้อสนอแนะเชิงนโยบายที่สำคัญได้แก่ 1. การเร่งตรวจสอบเพื่อหากลุ่มคนที่ยังเข้าไม่ถึงระบบประกันสุขภาพเนื่องจากขาดหลักฐานแสดงสิทธิโดยร่วมมือกับภาคประชาชนและองค์กรเอกชนในการค้นหากลุ่มคนดังกล่าว และในระหว่างนี้การให้บริการแก่คนเหล่านี้ไม่สามารถหลีกเลี่ยงได้จึงจำเป็นต้องมีการจัดสรรงบประมาณไปช่วยเหลือกลุ่มสถานพยาบาลเหล่านี้ 2. เร่งการประเมินและพัฒนาสถานพยาบาลในทุกระดับทั้งของรัฐและเอกชน 3. ประชาชนต้องเข้าร่วมในการบริหารจัดการในทุกระดับ ตั้งแต่การกำหนดนโยบาย การบริหารจัดการระบบโดยทำหน้าที่เป็นผู้ซื้อบริการระดับท้องถิ่น เข้าร่วมในคณะกรรมการบริหารสถานพยาบาล การเป็นผู้ให้บริการโดยเฉพาะบริการด้านการส่งเสริมสุขภาพ การติดตามตรวจสอบคุณภาพบริการ ความครบถ้วนของบริการตามชุดสิทธิประโยชน์ การจัดการข้อร้องเรียน การเผยแพร่ข้อมูลที่จำเป็นให้กับประชาชนโดยเฉพาะในเขตพื้นที่ห่างไกล โดยรัฐสนับสนุนการรวมกลุ่มของประชาชนในชุมชนให้เป็นผู้ทำหน้าที่ดังกล่าว 4. ข้อมูลที่จำเป็นที่จะต้องเผยแพร่สู่ประชาชนได้แก่ ข้อมูลในการเลือกสถานพยาบาล ข้อมูลในการตัดสินใจรับบริการการแพทย์ และข้อมูลเพื่อเพิ่มความรู้ความเข้าใจในระบบประกันสุขภาพและการใช้บริการที่ถูกต้อง
