The characteristics of problem in health service utilization under different health insurance schemes Abstract A summary of the resource. ลักษณะปัญหาการรับบริการสุขภาพภายใต้ระบบประกันสุขภาพต่าง ๆ การวิจัยนี้มีวัตถุประสงค์เพื่อ 1) วิเคราะห์สภาพปัญหาของผู้รับบริการภายใต้ระบบประกันสุขภาพ 5 ระบบที่มีอยู่ในปัจจุบันและผู้รับบริการที่อยู่นอกระบบประกันสุขภาพ 2) วิเคราะห์สาเหตุของปัญหาด้านการดำเนินการที่เป็นปัญหาต่อผู้รับบริการ 3) วิเคราะห์แบบแผนการใช้บริการสุขภาพของประชาชนที่ไม่มีหลักประกันสุขภาพ 4) วิเคราะห์ปัญหาการรับบริการซึ่งเกิดจากจุดอ่อนของระบบและคุณภาพการให้บริการ และ 5) เสนอแนะแนวทาง/กลไกที่จะนำไปสู่การปรับระบบการบริหารจัดการบริการสุขภาพระดับชาติ วิธีการวิจัยเป็นการผสมผสานระหว่างการวิจัยเชิงปริมาณและเชิงคุณภาพ การวิจัยเชิงปริมาณเก็บข้อมูลจากผู้รับบริการภายใต้ระบบประกันสุขภาพ 5 ระบบๆ ละ 400 ราย และผู้รับบริการที่ไม่มีหลักประกันสุขภาพ 400 ราย รวม 2,400 ราย ในจังหวัดสมุทรปราการ เชียงใหม่ ปัตตานี และศรีสะเกษ การวิจัยเชิงคุณภาพรวบรวมข้อมูลจากการจัดกลุ่มเสวนา 9 กลุ่ม กรณีศึกษา 12 กรณี และการสัมภาษณ์ผู้ทรงคุณวุฒิด้านบริการสุขภาพและผู้บริหารระดับสูง 9 ท่านผลการศึกษาซึ่งตอบวัตถุประสงค์ที่ตั้งไว้ สรุปได้ดังนี้ผู้รับบริการในทุกระบบประกันประสบปัญหาเมื่อไปรับบริการเป็นต้นว่า เจ้าหน้าที่ไม่ใคร่กระตือรือร้นในการให้บริการ ต้องรอแพทย์นาน แพทย์ใช้เวลาในการตรวจน้อย และไม่ให้คำอธิบายเกี่ยวกับความเจ็บป่วย สถานพยาบาลที่กำหนดให้ไม่สะดวกในการเดินทาง ระเบียบการส่งต่อผู้ป่วยยุ่งยาก มีความวิตกกังวลในคุณภาพของยาที่ได้รับ ไม่มั่นใจในการวินิจฉัยโรค ผู้รับบริการทุกระบบมีค่าใช้จ่ายนอกเหนือจากค่ารักษาพยาบาล ถ้าไปรับการรักษาโดยไม่แสดงบัตรก็มีฐานะเช่นเดียวกับผู้ไม่มีหลักประกันสุขภาพ ผู้ไม่มีหลักประกันสุขภาพที่เจ็บป่วยเรื้อรังไม่สามารถเข้าถึงระบบสุขภาพ ส่วนใหญ่จะเดือดร้อนเพราะค่าใช้จ่ายสูงเกินความสามารถในการจ่ายปัญหาด้านการดำเนินการที่มีผลกระทบต่อผู้รับบริการ เกิดจากการที่เจ้าหน้าที่ออกบัตรไม่ได้อธิบายให้เข้าใจสิทธิประโยชน์ของแต่ละระบบประกัน เจ้าหน้าที่ไม่เข้าใจสิทธิของผู้มีหลักประกันสุขภาพ งบประมาณต่อหัวที่ได้รับจากรัฐสำหรับผู้ป่วยบัตร สปร.น้อย ทำให้สถานบริการมีความลำเอัยงในการให้บริการโดยให้น้อยเกินไปสำหรับผู้ใช้สิทธิบางระบบที่เบิกได้น้อย และมากเกินไปสำหรับผู้ใช้สิทธิบางระบบที่เบิกได้มาก หรือปฏิเสธผู้ป่วยที่ถูกส่งตัวมารับการรักษาเนื่องจากปัญหาในการเบิกเงินระหว่างสถานพยาบาลซึ่งมีการค้างจ่ายมาก ผู้รับบริการได้รับผลกระทบจากการที่แพทย์และเจ้าหน้าที่ได้ค่าตอบแทนน้อยไม่เป็นแรงจูงใจในการให้บริการซึ่งเชื่อมโยงไปถึงคุณภาพของบริการประชาชนที่ไม่มีหลักประกันสุขภาพส่วนหนึ่งมีศักยภาพในการจ่ายค่ารักษาพยาบาลได้เอง แต่ส่วนใหญ่จะเป็นผู้ด้อยโอกาสที่ไม่สามารถจ่ายได้เลยหรือจ่ายได้เพียงบางส่วน แบบแผนการใช้บริการของประชาชนกลุ่มนี้ ถ้าเจ็บป่วยไม่รุนแรงหรือเริ่มเจ็บป่วยจะไปซื้อยากินเอง หรือไปคลินิกแพทย์ การใช้บริการโรงพยาบาลจะน้อยกว่าประชาชนที่มีหลักประกันคุณภาพ ถ้าเป็นผู้ป่วยนอกค่าใช้จ่ายไม่สูงจะพอพึ่งพาตนเองได้ แต่ถ้าเป็นผู้ป่วยในเตียงสามัญ พยาบาลมักแนะนำให้ซื้อบัตรสุขภาพหรือให้ไปพบนักสังคมสงเคราะห์ หากมีคุณสมบัติเข้าเกณฑ์หลักประกันสุขภาพได้ นักสังคมสงเคราะห์จะแนะนำให้ไปติดต่อหน่วยงานที่เกี่ยวข้องระบบประกันสุขภาพแต่ละระบบมีจุดอ่อนที่ต้องแก้ไข ในภาพรวมการซ้ำซ้อนของสิทธิและการขาดความต่อเนื่องของการมีสิทธิในหลักประกันทำให้การเก็บข้อมูลที่ถูกต้องเป็นเรื่องยากและเป็นอุปสรรคต่อการวางแผนที่มีประสิทธิภาพ งบประมาณที่ไม่เพียงพอมีผลกระทบต่อมาตรฐานของสถานพยาบาล กลไกในการจ่ายเงินเป็นปัจจัยสำคัญที่มีผลต่อพฤติกรรมของฝ่ายผู้ให้บริการ ขณะที่การใช้บริการโดยไม่ต้องจ่ายเงินเองทำให้ผู้รับบริการมีแนวโน้มใช้บริการเกินความจำเป็นคณะผู้วิจัยเห็นว่าข้อเสนอที่จะมีระบบประกันสุขภาพระบบเดียวสำหรับประชาชนทุกคนอาจเป็นไปได้ยากเมื่อพิจารณาตามสภาพการณ์ของประเทศไทย เนื่องจากความสามารถในการเสียภาษีและเก็บภาษีของเราไม่ทัดเทียมกับประเทศอื่นที่ประสบความสำเร็จจากการใช้ระบบนี้ สิ่งที่ควรเร่งรีบดำเนินการคือการปรับปรุงระบบบริหารจัดการด้านการเงินการคลังโดยให้ทุกคนมีสิทธิเข้าถึงบริการสุขภาพที่มีความจำเป็นโดยไม่เสียค่าใช้จ่าย แต่ให้รับผิดชอบค่าใช้จ่ายเองสำหรับบริการที่ไม่จำเป็น สำหรับประชาชนที่ไม่สามารถรับภาระค่าใช้จ่ายให้มีช่องทางการเข้าถึงบริการชุมชนซึ่งรัฐอาจให้การสนับสนุนในระยะเริ่มแรก
