The study of healthy Thai indicators Abstract A summary of the resource. การศึกษาเพื่อพัฒนาเครื่องชี้วัดคนไทยสุขภาพดี มีวัตถุประสงค์ที่สำคัญคือการสร้างภาพลักษณ์ของคนไทยที่มีสุขภาพดีพร้อมทั้งร่างกาย จิตใจ สังคม และพฤติกรรมที่จะนำไปสู่การมีสุขภาวะในช่วงอายุต่อๆไปด้วยรวมทั้งการพัฒนาเครื่องชี้วัดที่จะวัดคุณสมบัติเหล่านี้ด้วย การพัฒนาได้เริ่มจากการศึกษาปัญหาสุขภาพที่สำคัญในแต่ละกลุ่มอายุของคนไทย ปัจจัยเสี่ยงหรือสาเหตุของปัญหาและปัจจัยคุ้มครองซึ่งได้สร้างเป็นเกณฑ์และเครื่องชี้วัดคนไทยสุขภาพดีตามอายุและเพศ รวมทั้งสิ่งแวดล้อมที่สนับสนุนสภาวะสุขภาพด้วย จากนั้นขอความเห็นและข้อแนะนำจากผู้ทรงคุณวุฒิด้านต่างๆที่เกี่ยวข้องรวม 32 ท่าน รวมทั้งเสนอต่อกลุ่มต่างๆในชุมชน เช่น กลุ่มผู้สูงอายุ, กลุ่มแม่บ้าน, ผู้นำชุมชน ฯลฯ เพื่อรับฟังความคิดเห็น และนำมารวบรวมเป็นเกณฑ์และเครื่องชี้วัดคนไทยสุขภาพดีในกลุ่มอายุตั้งแต่ วัยทารกและก่อนวัยเรียน (0-5 ปี) วัยเรียน (6-12 ปี) วัยรุ่น (13-18 ปี) วัยทำงาน (19-59 ปี) และวัยสูงอายุ (60 ปีขึ้นไป) รวมทั้งสิ่งแวดล้อมที่สนับสนุนการมีสุขภาพดี คือคู่ครอง ครอบครัว โรงเรียน สถานที่ทำงานและชุมชน ได้นำเครื่องชี้วัดไปทดสอบความแม่นตรง และความเชื่อถือได้ พบว่าสามารถให้ความแม่นตรงในด้าน Content และ Constructed validity และมีความเชื่อถือได้อยู่ในระดับ 0.8 – 0.9 จากนั้นได้ปรับเครื่องชี้วัดเป็นแบบสอบถามเพื่อนำไปประเมินสุขภาวะของประชากรในจังหวัด 4 ภาคของประเทศไทย คือ กรุงเทพมหานคร, น่าน, สระแก้ว, อำนาจเจริญ และยะลา รวมประชากรที่สำรวจ 13,803 คน ใน 3919 ครอบครัว ผลการสำรวจในภาพรวมแสดงให้เห็นว่าประชาชนไทยในทุกกลุ่มอายุมีสุขภาพด้านกายภาพรวมทั้งการได้รับบริการตรวจรักษาอยู่ในระดับใช้ได้ แต่ระดับสติปัญญา (IQ) ในเด็กนักเรียนประมาณครึ่งหนึ่ง ยังอยู่ในระดับต่ำกว่ามาตรฐาน นอกจากนี้ด้านพฤติกรรมอนามัย และพฤติกรรมที่ลดความเสี่ยงจากอุบัติภัย ยังมีประชากรในสัดส่วนที่น้อย (ร้อยละ 30 – 40) ที่ปฏิบัติได้อย่างครบถ้วนถูกต้อง ด้านความฉลาดทางอารมณ์มีแนวโน้มว่าจะมีปัญหาเกี่ยวกับ ความรับผิดชอบ ความพอใจในชีวิต และความสุขสงบทางใจ ในเด็กก่อนวัยเรียน (3 ขวบขึ้นไป) มีประมาณร้อยละ 30 ที่เริ่มใช้ความรุนแรงคือการรังแกเพื่อนและทำร้ายสัตว์ต่างๆ เป็นประจำซึ่งสอดคล้องกับครอบครัวร้อยละ 40 ที่ยอมรับว่ามีการใช้ความรุนแรงทางวาจาหรือทำร้ายร่างกายซึ่งกันและกันอยู่ ในโรงเรียนมีการสอนทักษะชีวิตในการแก้ปัญหา การปฏิเสธโดยไม่ใช้ความรุนแรงเพียงร้อยละ 58.8 ของโรงเรียนทั้งหมด ในสถานที่ทำงานมีเพียงครึ่งหนึ่งเท่านั้นที่ร้านค้าในสถานที่นั้นๆไม่จำหน่ายบุหรี่ และเครื่องดื่มแอลกอฮอล์ สำหรับชุมชนด้านกายภาพ เช่นถนน สาธารณูปโภคมีความครอบคลุม ร้อยละ 80 ขึ้นไป แต่ที่ยังพบน้อยคือระบบป้องกันอุบัติภัย การรักษาความปลอดภัยและสถานที่พักผ่อนหย่อนใจร่วมกันของชุมชน ผลการสำรวจแสดงให้เห็นว่า เครื่องชี้วัดเหล่านี้สามารถแสดงสถานะสุขภาพในทางบวกของชุมชนได้ในระดับหนึ่งแต่ยังต้องการการศึกษาวิจัยต่อไปถึงความสามารถในการทำนายสุขภาพ (Predictive validity) และการปรับการวัดให้เป็นเชิงองค์รวมมากขึ้น นอกจากนี้กลุ่มผู้วิจัยยังได้ปรับเครื่องชี้วัดให้สั้นและง่ายขึ้นเหมาะสำหรับการใช้โดยประชากรหรือองค์กรชุมชนเพื่อประเมินสุขภาวะได้ด้วยตนเอง http://digital.nlt.go.th/dlib/files/original/92073136d79fb494f9e5fa78facc89ea.pdf 4effcfbb06aeaa75e63724434f2bfba6 Dublin Core The Dublin Core metadata element set is common to all Omeka records, including items, files, and collections. Macro-economic indices for measuring equity in health finance and delivery 1986-1998 Abstract A summary of the resource. ดัชนีเศรษฐมหภาคเพื่อวัดความเป็นธรรมทางการคลังและการใช้บริการสาธารณสุข ระหว่างปี 2529-2541 เมื่อนโยบายสาธารณสุขวางไว้อย่างชัดเจนว่าต้องการบรรลุความเป็นธรรมทางสุขภาพ ดัชนีวัดความเป็นธรรมก็จะมีความสำคัญในการเฝ้าติดตามเป้าหมายของนโยบาย กระแสความตื่นตัวด้านความเป็นธรรมทางสุขภาพมีอย่างน้อย 3 ทาง ได้แก่ กระแสทางสาธารณสุขศาสตร์ เศรษฐศาสตร์ และสังคมศาสตร์ ในที่นี้ เสนอการใช้ดัชนีทางเศรษฐศาสตร์มหภาค เพื่อเฝ้าติดตามความเป็นธรรมทางสุขภาพ ซึ่งมีทั้ง 2 มิติ คือ ความเป็นธรรมของการคลังสุขภาพ และความความเป็นธรรมของการใช้บริการ การตีค่าความเป็นธรรมของการคลังคือ การจ่ายตามกำลังความสามารถ ดัชนีคัควานีความก้าวหน้าของแหล่งการคลัง จะสะท้อนว่า วิธีทางการคลังนั้น มีความก้าวหน้าหรือถดถอยต่อรายได้ ถ้าถดถอยต่อรายได้แสดงว่า ไม่ได้เป็นไปตามหลักการจ่ายตามกำลังความสามารถ ส่วนการตีค่าความเป็นธรรมของการใช้บริการคือได้รับบริการตามความจำเป็นทางสุขภาพ ผู้ใดที่เจ็บป่วยมาก ผู้นั้นควรได้รับบริการสุขภาพมากกว่า ดัชนีการกระจายของการเจ็บป่วยและการใช้บริการ จะนำมาวัดความเป็นธรรมของการใช้บริการสุขภาพใช้ข้อมูลการสำรวจสภาวะเศรษฐกิจและสังคมครัวเรือนและการสำรวจอนามัยและสวัสดิการครัวเรือนของสำนักงานสถิติแห่งชาติ ระหว่างปี 2529-2541 เป็นหลักในการคำนวณดัชนีดังกล่าว เพื่อให้เห็นแนวโน้มของความเป็นธรรมทั้งด้านการคลังและการรับบริการ การสำรวจอนามัยและสวัสดิการครัวเรือน ไม่มีตัวแปรรายได้ จึงใช้ตัวแปรการศึกษา อาชีพ และภูมิลำเนาของหัวหน้าครัวเรือน ในการสร้างรายได้ให้ตรงกับข้อมูลรายได้ในการสำรวจสภาวะเศรษฐกิจและสังคมผลการศึกษาพบว่า ดัชนีคัควานีของการคลังสุขภาพมีลักษณะถดถอยต่อรายได้โดยตลอด ตั้งแต่ปี 2529-2541 แนวโน้มการถดถอยลดลงจนถึงปี 2539 และกลับเพิ่มขึ้นในปี 2541 การถดถอยเป็นเพราะ ดัชนีจินีของการกระจายรายได้มีความเหลื่อมล้ำเพิ่มขึ้นในระยะเวลาดังกล่าว นอกจากนี้ โครงสร้างแหล่งการคลังสาธารณะเพื่อการสุขภาพ ทั้งระบบภาษีโดยรวมและระบบประกันสังคม ยังมีลักษณะถดถอยต่อรายได้อยู่ การคลังเอกชนที่มาจากการเก็บเงิน ณ จุดรับบริการ มีลักษณะถดถอยต่อรายได้อย่างแน่นอน ดัชนีการกระจายของการเจ็บป่วย ส่วนใหญ่จะมีลักษณะถดถอยต่อรายได้ คือ คนจนรายงานว่าเจ็บป่วยมากกว่าคนรวย ส่วนดัชนีการกระจายของการรับบริการสุขภาพ (ตั้งแต่การซื้อยากินเอง ไปสถานีอนามัย คลินิก โรงพยาบาลรัฐและเอกชน) เมื่อเปรียบเทียบกับการเจ็บป่วยแล้ว พบว่า มีลักษณะก้าวหน้าต่อรายได้ คือ คนรวยมีโอกาสใช้บริการสุขภาพมากกว่าคนจน เมื่อเทียบกับการเจ็บป่วยหรือความจำเป็นทางสุขภาพที่เท่ากัน การปรับค่ามาตรฐานเพื่อลดความแตกต่างของตัวแปรโครงสร้างอายุและเพศ ทำให้ลักษณะความไม่เป็นธรรมลดลงเล็กน้อย ข้อเสนอแนะ ควรวางเป้าหมายในการปรับความเป็นธรรมทางสุขภาพให้ชัดเจน ข้อสรุปที่ชัดเจนจากการวิจัยนี้คือ ความไม่เป็นธรรมของการคลังสุขภาพ ควรจะลดลงด้วยการกระจายรายได้ที่เป็นธรรมมากขึ้น ระบบภาษีและเบี้ยประกันสังคมที่ก้าวหน้า ลดสัดส่วนของการจ่ายเงิน ณ จุดรับบริการ ส่วนความเป็นธรรมของการรับบริการให้ปรับระบบข้อมูลให้มีความตรงมากยิ่งขึ้น
