The Research and Development of Hypertensive Patient Service System at Health Centers in Phon District, Khon Kaen, Thailand Abstract A summary of the resource. การวิจัยและพัฒนาระบบบริการผู้ป่วยความดันโลหิตสูง ที่สถานีอนามัยตำบล อำเภอพล จังหวัดขอนแก่น การวิจัยครั้งนี้เป็นการวิจัยปฏิบัติการ เพื่อศึกษาคุณภาพของการให้บริการ ภายใต้การมีส่วนร่วมของผู้เกี่ยวข้อง และพัฒนาระบบบริการผู้ป่วยความดันโลหิตสูงที่เหมาะสมกับสถานีอนามัย ดำเนินการเป็น 3 ระยะ คือ ระยะก่อนปฏิบัติการ ปฏิบัติการ และวิเคราะห์สรุป โดยคัดเลือกกลุ่มผู้ป่วยความดันโลหิตสูงระดับรุนแรงน้อยถึงปานกลาง (รักษาด้วย Hydrochlorothiazide และ/หรือ Methyldopa) ขึ้นทะเบียนรักษาที่โรงพยาบาลพล สมัครใจรับการรักษาต่อ ณ สถานีอนามัย และโรงพยาบาลแห่งละ 43 คน ในช่วงเดือน เมษายน-กันยายน พ.ศ.2541 ทำการเก็บข้อมูลเชิงคุณภาพด้วยวิธีการสนทนากลุ่ม การสัมภาษณ์เชิงลึก การสังเกต และวิเคราะห์โดยการวิเคราะห์เชิงเนื้อหา ส่วนข้อมูลเชิงปริมาณ ใช้วิธีสำรวจคุณลักษณะของกลุ่มผู้ป่วย วัดระดับความดันโลหิต บันทึกการมาตามนัด และค่าใช้จ่ายตลอดการวิจัย แล้ววิเคราะห์โดยใช้สถิติเชิงพรรณนา สถิติวิเคราะห์ t-test และ chi-square ตามลำดับ สรุปผลการวิจัย ในด้านต่าง ๆ ได้แก่ 1. ลักษณะทั่วไปของผู้ร่วมวิจัย ส่วนใหญ่เป็นเพศหญิงมากกว่าร้อยละ 70 มีอายุตั้งแต่ 50 ปีขึ้นไป สถานภาพสมรสคู่ และอยู่ด้วยกันเป็นส่วนมาก 2. รูปแบบการให้บริการผู้ป่วยความดันโลหิตสูงที่สถานีอนามัย จากระบบเดิมที่รวมศูนย์การรักษาที่โรงพยาบาล ได้มีการปรับเปลี่ยน โดยผู้ป่วยที่รับการรักษาจนสามารถควบคุมความดันโลหิตสูงอยู่ในระดับที่ยอมรับได้ ตามหลักเกณฑ์การส่งต่อ จะถูกส่งไปรับบริการที่สถานีอนามัยใกล้บ้าน และมีการส่งกลับมาโรงพยาบาลเมื่อมีข้อบ่งชี้ทางภาวะแทรกซ้อน 3. คุณภาพของการให้บริการผู้ป่วยความดันโลหิตสูง ด้านการควบคุมระดับความดันโลหิต, ความร่วมมือในการรักษา, ค่าใช้จ่ายเมื่อไปรับบริการ และผลข้างเคียงจากการรับประทานยา 4. ความพึงพอใจของผู้ป่วยความดันโลหิตสูงต่อการมารับบริการที่สถานีอนามัย 4.1 จากการสัมภาษณ์ พบว่า ระดับความพึงพอใจต่อการมารับบริการใน 5 ด้าน คือ ความสะดวกที่ได้รับ, อัธยาศัย ความสนใจของเจ้าหน้าที่, สถานที่ เครื่องมืออุปกรณ์, คุณภาพบริการ และค่าใช้จ่าย อยู่ในเกณฑ์พึงพอใจมาก 4.2 จากการสนทนากลุ่ม พบว่า โดยส่วนรวมผู้ป่วยมีความพึงพอใจต่อระบบบริการที่สถานีอนามัย นอกจากนั้น ผู้ป่วยรู้สึกว่าการเดินทางไปรับบริการที่สถานีอนามัยปลอดภัยมากกว่าการมาโรงพยาบาลล ตลอดจนสามารถปฏิบัติตามคำแนะนำได้ดีข้น http://digital.nlt.go.th/dlib/files/original/1baad84daf58134e1e1db74d469a6a00.pdf 8133f14c387f681af0a50efb8fc78cf2 Dublin Core The Dublin Core metadata element set is common to all Omeka records, including items, files, and collections.
