A community survey of musculoskeletal pain and manangement in Namphong district, Khon Kaen province Abstract A summary of the resource. โรคปวดกล้ามเนื้อ ปวดข้อ และการใช้ยาชุดของประชาชน อำเภอน้ำพอง จังหวัดขอนแก่นวัตถุประสงค์ : เพื่อศึกษาอุบัติการและสาเหตุของ การปวดกล้ามเนื้อและข้อ วิธีการดูแลตนเองเมื่อมีอาการปวด และการกระจายของยาชุดแก้ปวดกล้ามเนื้อในชุมชน เขตอำเภอน้ำพอง จังหวัดขอนแก่นรูปแบบการศึกษา : เป็นการศึกษาเชิงสำรวจพรรณนา (Descriptive survey study)สถานที่ทำการศึกษา : ชุมชนเขตอำเภอน้ำพอง จังหวัดขอนแก่นกลุ่มตัวอย่าง : -แหล่งจำหน่ายยาชุด 86 แห่ง ในหมู่บ้าน 12 หมู่บ้านที่ได้รับการสุ่มเลือกจากหมู่บ้านทั้งหมดของอำเภอน้ำพองโดยวิธีการสุ่มอย่างง่าย ประชากรที่มีอายุตั้งแต่ 15 ปีขึ้นไป ที่อาศัยอยู่ในหมู่บ้านที่ได้รับการสุ่มเลือกทั้ง 12 แห่งจำนวน 685 คน (ทำการสุ่มเลือกมาแบบ Cluster randomization โดยใช้หลังคาเรือนเป็นหน่วยสุ่ม)วิธีการ : -ทำการล่อซื้อยาชุดจากแหล่งจำหน่ายในหมู่บ้านที่ได้รับการสุ่มเลือก แล้วทำการสำรวจโดยเปิดเผยซ้ำ นำยาที่ได้มาทั้งหมดไปวิเคราะห์ชนิดของยา เปรียบเทียบยาชุดที่ได้จากการสำรวจทั้งสองวิธี -สัมภาษณ์กลุ่มตัวอย่างถึงอาการปวดและพฤติกรรมการรักษาตนเอง แล้วทำการตรวจร่างกายผู้ที่มีอาการปวดภายใน7วันก่อนการให้สัมภาษณ์เพื่อหาสาเหตุของอาการปวดโดยใช้เกณฑ์การวินิจฉัยโรคมาตรฐานที่กำหนดโดย American Rheumatism Associationผลการวิจัย : มีการจำหน่ายเพื่อบรรเทาอาการปวดกล้ามเนื้อและข้อในร้านชำทุกร้านของหมู่บ้าน จำนวน 68 แห่ง และพบแหล่งขายตรงจากการล่อซื้ออีก 18 แห่ง พบยาชุดทั้งสิ้น 51 รูปแบบ เมื่อทำการวิเคราะห์ตัวยาพบว่าในจำนวนนี้ร้อยละ 10 มี corticosteroid เป็นส่วนประกอบอย่างเดียว ร้อยละ 16 มีเฉพาะ NSAIDs เป็นส่วนประกอบ และร้อยละ 71 มียาทั้งสองกลุ่มเป็นส่วนประกอบ corticosteroid ที่พบทั้งหมดอยู่ในรูปของ dexamethasone ขนาด 1-3 เม็ดต่อชุด ส่วน NSAIDs พบIndomethacin และ Phenylbutazone เป็นส่วนใหญ่ ประชากรกลุ่มตัวอย่างร้อยละ 74 เคยมีปัญหาเรื่องปวดกล้ามเนื้อและข้อ ผู้ที่มีอาการปวดภายใน7วันพบถึงร้อยละ43 สาเหตุของอาการปวดส่วนใหญ่เกิดจากการทำงาน ภาวะที่พบมากที่สุดคือปวดหลังส่วนล่างพบร้อยละ 54 โดยมีสาเหตุมาจากกล้ามเนื้อหลังยอกและข้อต่อกระดูกสันหลังเสื่อมมากที่สุด ภาวะที่พบรองลงมาได้แก่ เอ็นรอบข้ออักเสบร้อยละ 25 myofacial pain syndromeร้อยละ 17 ข้อเสื่อมร้อยละ 17 โรคข้ออักเสบเรื้อรังที่ต้องการการรักษาต่อเนื่องพบเพียงร้อยละ 9 จากการสัมภาษณ์ผู้ที่เคยมีอาการปวดทั้งหมด 508 ราย พบว่าร้อยละ 58 เคยกินยาชุด ส่วนใหญ่กินเป็นครั้งคราวเมื่อเกิดอาการ มีผู้ที่กินยาชุดเป็นประจำมีร้อยละ12.5 ในจำนวนนี้ 1 ใน 4 ยอมรับว่ากินยาชุดทุกวัน กลุ่มตัวอย่างกว่าร้อยละ 95 ทราบว่ายาชุดมีอันตรายโดยมีแหล่งข้อมูลที่สำคัญคือ สื่อโฆษณาและเจ้าหน้าที่สาธารณสุข แต่เมื่อมีอาการปวดร้อยละ 68 เลือกที่จะรักษาตัวเอง โดยร้อยละ33 ซื้อยาชุดกินเองแม้จะทราบว่ายาชุดมีอันตรายเนื่องจากสะดวก หาง่าย และราคาถูก ปัจจัยที่มีอิทธิพลต่อการเลือกวิธีรักษาของกลุ่มตัวอย่างมากที่สุดได้แก่ประสบการณ์ในอดีตและคำแนะนำจากญาติและเพื่อน วิธีการรักษาอื่นๆนอกเหนือจากการใช้ยาชุดที่เป็นที่นิยมมากที่สุดได้แก่การนวดและการประคบร้อน รองลงมาได้แก่การไปรับการรักษาตามสถานพยาบาลแผนปัจจุบัน แต่มีเพียงร้อยละ 6.7 เลือกไปสถานีอนามัยทั้งที่เป็นสถานพยาบาลที่ใกล้บ้านมากที่สุดสรุปผลและข้อเสนอแนะ : ยาชุดซึ่งมียาอันตรายเป็นส่วนประกอบยังเป็นปัญหาของชุมชน มีการกระจายของยาอยู่ทั่วไปในหมู่บ้าน ประชาชนหาซื้อได้ง่ายและเป็นที่นิยมแม้ประชาชนส่วนใหญ่จะทราบถึงอันตรายของยาชุดก็ตาม อาการปวดข้อและกล้ามเนื้อที่พบในชุมชนส่วนใหญ่เกี่ยวเนื่องกับการทำงานและใช้เวลาในการรักษาไม่นาน โรคทางรูมาติซั่มที่เรื้อรังและรักษายากพบได้น้อยมากในชุมชน เมื่อเกิดปัญหาเกี่ยวกับการปวดกล้ามเนื้อและข้อประชาชนมักเลือกวิธีที่สะดวก ราคาถูกและสอดคล้องกับวิถีชีวิตของตนเอง วิธีการแก้ปัญหาการใช้ยาชุดแก้ปวดเมื่อยในชุมชนควรให้ทางเลือกในการรักษาที่เน้นความสะดวก ง่าย ปลอดภัยและมีประสิทธิภาพเช่น การแพทย์แผนไทย การนวดและการประคบ ควบคู่ไปกับการพัฒนาศักยภาพของเจ้าหน้าที่สถานีอนามัยให้สามารถดูแลรักษาโรคปวดกล้ามเนื้อและปวดข้อที่พบบ่อยในชุมชนได http://digital.nlt.go.th/dlib/files/original/b11a33f0cf4d89cebd8b9fadc7724500.pdf 39506c6166abc48d59b8322ad35aa89d Dublin Core The Dublin Core metadata element set is common to all Omeka records, including items, files, and collections.
